This position offers an opportunity to make a meaningful difference in patients’ lives while working within a dynamic, supportive environment committed to excellence in patient-centered care. *<Responsibilities>* * Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. * Monitor and record vital signs, including blood pressure, pulse, respiration, and temperature, accurately documenting all patient information in electronic health records (EHR) systems. * Provide emotional support and companionship to patients and their families, respecting cultural and individual preferences. * Support medication administration under supervision, ensuring proper handling and documentation in compliance with HIPAA regulations. * Assist with wound care, catheter management, and other basic medical procedures following aseptic techniques. * Collaborate closely with interdisciplinary teams—including nurses, social workers, and physicians—to coordinate comprehensive hospice care plans. * Maintain a safe environment by adhering to infection control protocols and safety standards across all patient interactions. *<Skills>* * Proven experience in hospice or palliative care settings with knowledge of Alzheimer’s care, dementia care, memory care, and behavioral health needs. * Familiarity with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks for accurate medical documentation. * Experience working in diverse environments including long-term care facilities, nursing homes, assisted living communities, hospitals (including ICU and Level I Trauma Centers), outpatient clinics, or home & community care settings. * Strong understanding of physiology, anatomy, medical terminology, and vital signs monitoring essential for patient care. * Ability to perform caregiving tasks involving mobility assistance using Hoyer lifts or other assistive devices safely. * Knowledge of infection control practices including aseptic technique and basic life support (BLS). * Excellent communication skills to effectively collaborate with healthcare teams and provide compassionate patient service.
