*<Overview>* Join our compassionate healthcare team as a Certified Nursing Assistant (CNA) specializing in hospice care. In this vital role, you will provide personalized, respectful support to patients nearing the end of life, ensuring comfort, dignity, and quality of life. This position offers an opportunity to make a meaningful difference in patients’ lives while working within a dynamic, supportive environment committed to excellence in patient-centered care. *<Responsibilities>* * Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. * Monitor and record vital signs, including blood pressure, pulse, respiration, and temperature, accurately documenting all patient information in electronic health records (EHR) systems. * Provide emotional support and companionship to patients and their families, respecting cultural and individual preferences. * Support medication administration under supervision, ensuring proper handling and documentation in compliance with HIPAA regulations. * Assist with wound care, catheter management, and other basic medical procedures following aseptic techniques. * Collaborate closely with interdisciplinary teams—including nurses, social workers, and physicians—to coordinate comprehensive hospice care plans. * Maintain a safe environment by adhering to infection control protocols and safety standards across all patient interactions. *<Skills>* * Proven experience in hospice or palliative care settings with knowledge of Alzheimer’s care, dementia care, memory care, and behavioral health needs. * Familiarity with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks for accurate medical documentation. * Experience working in diverse environments including long-term care facilities, nursing homes, assisted living communities, hospitals (including ICU and Level I Trauma Centers), outpatient clinics, or home & community care settings. * Strong understanding of physiology, anatomy, medical terminology, and vital signs monitoring essential for patient care. * Ability to perform caregiving tasks involving mobility assistance using Hoyer lifts or other assistive devices safely. * Knowledge of infection control practices including aseptic technique and basic life support (BLS). * Excellent communication skills to effectively collaborate with healthcare teams and provide compassionate patient service.
