Job description: *Reliant Home Health Care* *Position Summary* Reliant Home Health Care is seeking a reliable and compassionate *LVN* to provide skilled nursing care to patients in their homes. This role is responsible for completing weekend visit assignments, delivering hands-on patient care, and ensuring continuity of services when our weekday staff is off duty. The ideal candidate is detail-oriented, independent, and dedicated to supporting patients’ safety, comfort, and health. *Key Responsibilities* * Provide direct nursing care to patients in accordance with their individualized plan of care. * Perform weekend patient visits, including vital signs, medication administration, wound care, catheter care, and chronic condition monitoring. * Complete accurate and timely documentation of all patient care activities in the agency’s EMR system. * Observe, evaluate, and report changes in patient condition to the supervising RN and/or physician. * Assist patients and families with education on disease processes, medications, and home safety practices. * Follow established nursing procedures and agency protocols to ensure high-quality home health services. * Maintain professional communication with patients, families, and the care team. * Participate in on-call duties as assigned (if applicable to the weekend schedule). *Qualifications* * Current and active LVN license in the state of practice. * Minimum of 1 year of nursing experience (home health experience preferred). * Current CPR/BLS certification. * Reliable transportation, valid driver’s license, and auto insurance. * Strong clinical assessment skills within LVN scope of practice. * Ability to work independently and manage a weekend caseload. * Excellent documentation and communication abilities. *Preferred Skills* * Experience with home health electronic medical record systems. * Knowledge of wound care, diabetes management, and elderly care. * Bilingual abilities (optional). *Work Environment* * Field-based position involving patient home visits. * schedule with some flexibility based on patient needs.
