Our team of healthcare professionals works together to support patients with empathy, respect, and clinical excellence. *Position Summary:* We are seeking a dedicated and compassionate *Registered Nurse (RN)* to join our hospice team. The ideal candidate will provide skilled nursing care and coordinate the patient’s care in collaboration with the hospice interdisciplinary team. *Responsibilities:* * Provide direct nursing care to hospice patients in their homes, assisted living, or care facilities. * Assess physical, emotional, and spiritual needs of patients and families. * Develop and implement individualized care plans in collaboration with the interdisciplinary team. * Administer medications, treatments, and symptom management. * Educate patients and families on disease progression, comfort measures, and end-of-life care. * Coordinate with physicians, social workers, chaplains, and other team members to ensure holistic care. * Maintain accurate and timely documentation in accordance with hospice policies and regulations. * Participate in on-call rotation and respond to urgent patient needs as scheduled. * Ensure strict adherence to *HIPAA guidelines and patient confidentiality*. * Must be willingly to do patient admissions as needed. * Must have HOPE education and knowledge. *Qualifications:* * Current and valid Registered Nurse (RN) license in the state of California. * Current CPR certification. * Valid California driver’s license and reliable transportation. * Minimum 1 year of nursing experience (hospice or palliative care preferred). * Excellent communication and emotional support skills. * Ability to work independently and as part of a team. * Compassionate, dependable, and emotionally resilient in end-of-life care settings. *What We Offer:* * Flexible work schedule (Per Diem) * Supportive, compassionate team culture * Ongoing training and continuing education * Mileage reimbursement (After 20 miles) \*\*Please note that this is a *Per Visit* position, meaning you will be paid for the time spent during each patient visit. This is not a traditional shift-based role. Your schedule will vary depending on patient needs, and you will be compensated for each visit you make to the patient's home or care facility. _In addition, you will be compensated for attending __*in-service meetings*_ _and training sessions._ _*Pay starting at $70 per visit. *_ Pay: From $70.00 per hour Benefits: * Flexible schedule Work Location: In person
