*Job Overview:* We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. As a home health nurse, you will provide one-on-one care to patients in the comfort of their homes, focusing on personalized treatment plans, patient education, and improving health outcomes. You will work closely with physicians, therapists, and other members of the care team to ensure high-quality, patient-centered care. This position offers flexibility, autonomy, and the opportunity to make a meaningful difference in the lives of patients and their families. *Key Responsibilities:* * Conduct in-home assessments and develop individualized care plans. * Administer medications and treatments as prescribed. * Monitor patient progress and adjust care plans as needed. * Educate patients and families on disease processes, medications, and self-care techniques. * Document care provided in accordance with agency policies and regulatory standards. * Communicate effectively with physicians, case managers, and other healthcare providers. * Comply with infection control, safety, and privacy regulations. *Qualifications:* * Active RN license in the state of [Insert State]. * Minimum of 1 year of nursing experience (home health or acute care preferred). * CPR certification (BLS required). * Valid driver’s license and reliable transportation. * Strong organizational, clinical, and communication skills. * Ability to work independently and manage time effectively. * Knowledge of electronic medical records (EMR) systems is a plus. *Preferred:* * Experience with OASIS documentation. * Bilingual abilities (e.g., Spanish, Korean, Tagalog, etc.) are a plus. * Familiarity with Medicare guidelines and home health compliance standards.
