Location: Varied Locations Part-Time,Weekends This is a part time LVN position throughout our coverage area. Hours: Weekends a month, 08:00 am to 5:00 pm; 4 pm to Midnight Functions & Responsibilities: - Responds to inquiries and requests for hospice information in a timely manner - Explains the hospice program to patients, family members, and other referral sources in an accurate, complete and thorough manner, EOB's - Provides accurate and timely documentation of all consents to the Admission department. - Demonstrates ability to manage several responsibilities simultaneously and meets productive standards of the organization - Assignments are completed meeting standards such as accuracy, neatness, and thoroughness. - Demonstrates dependability by adhering to time frames, punctuality and attendance policies - Adapts to changing situations well and handles difficult situations in a tactful manner. - Provides direct patient care under the direction of Team Manager and Director of Patient Care Services. Qualifications: *Currently licensed as a LVN in California *Willingness to work in patients home in various conditions; ability to work flexible schedule and adapt to a change of assignment * Valid Driver's License and Automotive Insurance required * Valid CPR certification (American Heart Association) * Valid IV certification Education: Completion of course of study as required acquiring LVN license in the State of California. Graduate of an approved school of Practical/Vocational nursing required. IV Cert required
