*Shift:* Day 4x10-Hour (11:30 - 22:00) Day 4x10-Hour (08:30 - 19:00) *Job Summary* We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across a variety of settings, including long-term care facilities, assisted living communities, hospitals, and home health environments. Your expertise will help ensure patients receive personalized, effective treatment while supporting their comfort and well-being. This position offers an exciting opportunity to make a meaningful difference in patients' lives through skilled nursing care delivered with enthusiasm and professionalism. *Duties* * Administer medications, including injections and IV infusions, following physician orders and safety protocols. * Conduct comprehensive patient assessments by monitoring vital signs, performing physical examinations, and evaluating symptoms to inform treatment plans. * Assist with complex procedures such as catheterization, wound suturing, and tube feeding, ensuring aseptic techniques are maintained at all times. * Manage patient documentation accurately within Electronic Health Record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations. * Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments in memory or hospice care settings. * Support acute pain management strategies and participate in triage to prioritize patient needs effectively. * Collaborate with interdisciplinary teams in diverse environments such as ICU, emergency room, medical-surgical units, or outpatient clinics to deliver seamless patient care. *Experience* * Valid LPN/LVN license in good standing with relevant state licensing boards. * Prior experience working in critical care settings such as ICU or Level I Trauma Centers is highly preferred. * Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate medical documentation. * Experience providing care in various environments including hospital medicine, long-term care facilities, assisted living communities, or home health settings. * Knowledge of ICD coding (ICD-9/ICD-10), CPT coding, and medical terminology to support precise documentation and billing processes. * Hands-on experience with IV insertion, phlebotomy, medication administration, and infection control protocols. * Strong understanding of physiology, anatomy, and diagnostic evaluation techniques to perform thorough patient assessments and support treatment plans effectively.
