*Job Summary* We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to join our team as a Field Nurse, providing vital in-home and community-based nursing care. In this role, you will deliver comprehensive patient assessments, administer treatments, and coordinate care plans to promote health and well-being across diverse populations. Your expertise will help improve patient outcomes by offering compassionate, high-quality nursing services in various settings, including homes, assisted living facilities, and community clinics. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals requiring specialized care. *Responsibilities* * Conduct thorough patient assessments, including vital signs, physical examinations, and health histories to determine care needs. * Administer medications, injections, IV infusions, and other treatments following prescribed protocols while ensuring patient safety. * Manage complex cases involving dementia care, Alzheimer's support, memory care, and behavioral health needs with sensitivity and professionalism. * Perform specialized procedures such as wound care, and sterile techniques adhering to infection control standards. * Utilize electronic health record (EHR) systems like HospiceMD for accurate documentation of patient interactions, treatment plans, and progress notes. * Collaborate with interdisciplinary teams including physicians, case managers, and social workers to coordinate comprehensive care plans. * Provide patient education on medication administration, disease management (including infectious diseases), and health coaching to empower individuals in managing their health effectively. *Requirements* * Valid LVN license in the state of practice with current licensure and compliance with state regulations. * Demonstrated proficiency with EMR/EHR systems including, HospiceMD for medical documentation. * Strong knowledge of medical terminology * Experience working with diverse populations including pediatrics, geriatrics, individuals with disabilities or developmental disabilities. * Skilled in performing complex nursing procedures such as phlebotomy, venipuncture, sterile processing techniques, and airway management. * Ability to handle urgent situations confidently with acute pain management skills and emergency response readiness. * Excellent communication skills for triage assessments and patient education; ability to work independently in the field while maintaining safety standards.
