*Overview* Join our dedicated healthcare team as a Licensed Vocational Nurse (LVN) specializing in Home Health Care! In this vital role, you will provide compassionate, high-quality nursing services directly in patients’ homes, promoting independence and well-being. Your expertise will help deliver personalized care that makes a real difference in patients’ lives. This position offers an exciting opportunity to apply your clinical skills in a dynamic, patient-centered environment while working independently and as part of a supportive team. *Responsibilities* * Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner. * Administer medications, injections, IV infusions, and other treatments following strict protocols and HIPAA regulations to ensure patient safety. * Manage complex care needs including wound care, catheterization, tube feeding, and ventilator management, especially for patients with chronic conditions like dementia or Alzheimer's disease. * Coordinate discharge planning and collaborate with case management teams to ensure seamless transitions from hospital or facility to home care. * Conduct patient education on medication administration, disease management (e.g., infectious disease care), and health coaching tailored to individual needs. * Utilize critical thinking and triage skills to evaluate urgent situations and determine appropriate interventions promptly. * Maintain accurate medical documentation, medical records, and coding (ICD-9/10) to ensure compliance with healthcare standards and regulations. *Experience* * Valid LVN license with current state certification required; prior experience in home health care strongly preferred. * Experience working with diverse patient populations including seniors, individuals with developmental disabilities, or those requiring memory or dementia care. * Background in acute care settings such as ICU, emergency room, or hospital medicine—particularly Level I Trauma Center or Level II Trauma Center experience—is highly desirable. * Familiarity with medical procedures like phlebotomy, venipuncture, sterile technique, and specimen collection/processing is advantageous. * Knowledge of physiology, anatomy, pharmacology (including CPT coding), and medical terminology to support accurate patient assessment and treatment planning. * Ability to work independently in a home environment while maintaining high standards of infection control and aseptic technique.
