Responsibilities: - Provide direct patient care in various settings such as assisted living facilities and patient homes. - Conduct patient assessments and develop individualized care plans. - Monitor patient vital signs and provide appropriate interventions. - Assist with triage and prioritize patient needs. - Provide education and support to patients and their families. - Collaborate with healthcare team members to ensure comprehensive and coordinated care. - Document patient information accurately and timely. Requirements: - Valid LVN/LPN in the state of California - Proven experience in providing home health care, hospice care including experience with injections, medication teaching, vitals and wound care. - Knowledge of infection control - Ability to provide health teaching and assist patient in appropriate decisions regarding patient care. - Strong communication skills to interact with patients, families, and healthcare team members. - Proficient in administering medications via various routes (e.g., oral, inhalation). - Familiarity with electronic medical records systems. - Ability to work independently and prioritize tasks effectively.
