Job description: Job Overview We are seeking an LVN to join our team! You will be responsible for delivering high quality care to a patient in Long Beach, CA. The patient needs hourly shift care at their home. Knowledge of quadriplegic patients is preferred. Patience, kindness and a genuine attitude of compassionate care are a MUST. The shift opening is every Monday night from 11PM-7AM. Spanish-speaking is a PLUS. Responsibilities Follow the plan of care set by the RN Case Manager Administer medications and treatments as prescribed, including tube feeding and acute pain management interventions. Provide skilled nursing care in various settings, including home health, ensuring a safe and supportive environment for patients. Collaborate with physicians, social workers, and other healthcare professionals to coordinate comprehensive care for patients and their families. Educate patients and families about disease processes, treatment options, and end-of-life care decisions. Monitor patient conditions closely for any changes or complications related to infectious disease care or other health issues. Document all patient interactions accurately in compliance with regulatory standards. Qualifications Current LVN nursing license in the state of practice. Experience in primary care, urgent care, emergency room (ER), or laboratory settings is preferred. Proficiency in medical imaging techniques is a plus. Strong understanding of hospice philosophy and palliative care principles. Excellent communication skills to effectively interact with patients, families, and healthcare team members. Ability to handle emotional situations with sensitivity and professionalism. Must be able to work flexible hours as needed to meet patient needs.
