Join us to help create a caring community where every person feels valued and supported! *Responsibilities* * Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support * Monitor patient health by checking vital signs such as blood pressure, pulse, and temperature regularly * Administer medication as prescribed and maintain accurate medication administration records * Support clients with meal preparation and nutritional needs, ensuring dietary guidelines are followed * Provide companionship and emotional support to reduce feelings of loneliness or isolation * Implement care plans tailored to each individual’s unique needs, including dementia, Alzheimer’s, autism, or developmental disabilities * Use specialized equipment such as Hoyer lifts or resident lifts safely during transfers and mobility assistance * Maintain a clean and safe environment by assisting with light housekeeping tasks like cleaning and laundry * Document patient observations, progress notes, and any changes in condition accurately in compliance with HIPAA regulations * Collaborate with healthcare professionals to ensure comprehensive patient care across home health, hospice, assisted living, or group home settings *Experience* * Prior experience working with seniors or individuals with disabilities in home care, assisted living facilities, nursing homes, or group homes is highly preferred * Knowledge of patient observation techniques and monitoring vital signs accurately * Hands-on experience with medication administration and care planning is advantageous * Familiarity with HIPAA regulations to ensure patient confidentiality at all times * Experience providing Alzheimer’s care, dementia care, memory care or working with individuals with developmental disabilities or autism is a plus * Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary * Strong communication skills for working effectively with clients, families, and healthcare teams Join our team today to provide compassionate care that truly makes a difference!
