If you are energetic, caring, and eager to support others in a home-based setting, this role is perfect for you! *Duties* * Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support * Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration rates using EMR (Electronic Medical Record) systems * Support patients with medication administration following prescribed instructions while maintaining compliance with HIPAA regulations * Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and safety monitoring * Assist with catheter care, wound care, and other medical procedures under supervision or guidance from nursing staff * Help with meal preparation and nutritional needs while ensuring food safety standards are met * Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment for clients * Use assistive devices like Hoyer lifts or other mobility aids to ensure safe patient transfers and prevent injuries * Document patient progress accurately in medical records and communicate effectively with healthcare teams about changes in condition * Support hospice or palliative care patients by providing comfort measures and emotional support during difficult times *Qualifications* * Proven experience as a Home Health Aide or in a similar caregiving role within assisted living facilities, nursing homes, or home & community care settings * Knowledge of medical terminology and familiarity with EMR/EHR systems for documentation purposes * Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as medication administration or hospice care are a plus * Experience working with individuals with developmental disabilities or dementia is highly desirable * Ability to perform patient observation accurately and respond promptly to changing health conditions * Strong communication skills to effectively interact with patients, families, and healthcare professionals * Valid driver’s license and reliable transportation preferred for live-in employment or community visits * Commitment to maintaining patient confidentiality in accordance with HIPAA regulations Embark on a rewarding career where your compassion and skills directly improve the quality of life for those you serve.
