Hospice Certified Home Health Aide (CHHA)

Job Description

*Overview* As a Home Health Aide, you will play a crucial role in providing compassionate and personalized care to individuals in their homes or community settings. Your responsibilities will include assisting clients with daily living activities, ensuring their comfort and safety, and promoting their overall well-being. This position requires a dedicated individual who is passionate about enhancing the quality of life for those in need of support. *Duties* * Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting * Provide companionship and emotional support to clients * Monitor and document vital signs and changes in client health status * Administer medications as prescribed and ensure proper medication management * Assist with mobility using equipment such as Hoyer lifts when necessary * Perform light housekeeping tasks including cleaning, laundry, and meal preparation * Support clients with Alzheimer’s care, dementia care, and memory care techniques * Maintain accurate medical documentation in compliance with HIPAA regulations * Collaborate with healthcare professionals to develop and implement care plans tailored to individual needs * Provide hospice care and support for clients with terminal illnesses *Skills* * Strong patient observation skills to identify changes in health status * Experience in assisted living or nursing home environments * Familiarity with EMR/EHR systems for medical documentation * Knowledge of medical terminology and vital sign monitoring * CPR certification and experience with catheter care preferred * Compassionate caregiving skills for working with individuals with developmental disabilities or long-term care needs * Ability to prepare nutritious meals while considering dietary restrictions * Proficiency in cleaning tasks to maintain a safe and hygienic environment This position is ideal for individuals who are committed to providing high-quality home health care while fostering meaningful relationships with clients.
