*Overview* Join our dedicated healthcare team as a Certified Home Health Aide (CHHA), where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. In this vital role, you will provide personalized in-home support, ensuring safety, comfort, and dignity for individuals with diverse needs. This position offers an exciting opportunity to work closely with patients, families, and healthcare professionals to promote independence and enhance quality of life through attentive, professional caregiving. *Duties* * Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary. * Monitor patient health by observing vital signs and reporting changes promptly to healthcare providers. * Administer medications and perform catheter care following medical instructions and HIPAA regulations to ensure safety and compliance. * Support patients with memory care needs, including Alzheimer’s and dementia care, creating a safe environment tailored to their unique requirements. * Document all patient interactions accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, maintaining detailed medical records and documentation. * Assist with light housekeeping tasks such as cleaning, laundry, and meal preparation to promote a healthy living environment. * Provide companionship and emotional support while respecting patient privacy and dignity in home and community settings. *Experience* * Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred. * Demonstrated ability to perform patient observation and respond effectively to changing conditions. * Knowledge of medical terminology, medication administration protocols, and medical documentation standards. * Experience working with individuals with developmental disabilities or those requiring hospice or memory care services is advantageous. * Certification in CPR is required; additional certifications such as hospice care training or dementia care are a plus. * Familiarity with EMR/EHR systems for accurate recordkeeping; experience with home & community care settings is desirable. * Long-term care experience combined with strong caregiving skills ensures success in providing compassionate patient care across diverse populations.
