RN and LVN Home Health Nurse

*Job Overview* We are seeking a compassionate and dedicated Registered Nurses (RN) and Licensed Vocational Nurses (LVN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring they receive the best possible support for their health needs. This role requires a strong commitment to patient-centered care and the ability to work independently while managing a diverse caseload. *Responsibilities* * Conduct comprehensive patient assessments and develop individualized care plans. * Administer medications and treatments as prescribed, ensuring adherence to safety protocols. * Monitor patient progress and adjust care plans as necessary based on clinical evaluations. * Provide education and support to patients and their families regarding health management and disease prevention. * Collaborate with interdisciplinary teams to coordinate care and improve patient outcomes. * Document all patient interactions accurately in electronic health records * Assist with procedures such as spinal taps, when necessary, adhering to aseptic techniques. * Provide specialized care for patients with infectious diseases and those requiring pediatric support. * Support senior care initiatives and work with individuals with developmental disabilities. *Requirements* * Current Registered Nurse (RN) or Licensed Vocational Nurse (LVN) license in the state of practice. * Previous experience in home health, travel nursing, or relevant clinical settings preferred. * Knowledge of ICD-10 coding is advantageous. * Strong skills in aseptic technique and infection control practices. * Experience in senior care is a plus. * Ability to work independently and manage time effectively while traveling between patient homes. * Excellent communication skills for effective interaction with patients, families, and healthcare teams. * Must be willing to undergo background checks as required by healthcare regulations.
