You will be responsible for delivering high-quality care to assigned Hospice patients. Clinical experience is preferred and candidates with Hospice experience are highly desirable. Responsibilities: Provide Skilled Nursing Care in a home or facility setting Ensure the appropriate use of meds, DME, and supplies Educate and support patients, families, and caregivers Participate in IDT and updating the Plan of Care Provide our patients and families the best care possible Daily travel is required when working Qualifications: Valid LVN License in California One year or more of experience in either Home Health or Hospice is preferred, but not required Previous experience in a hospice setting is a plus Ability to build rapport with patients Compassionate and caring demeanor Excellent written and verbal communication skills
