Benefits: Competitive salary Opportunity for advancement Training & development We are seeking on-call, base pay plus per-visit LVN Licensed Vocational Nurse or Registered Nurse to join our team in the OC/LA area! You will be responsible for the weekend from 5pm on Friday until 8am on Monday morning. Clinical hospice experience is required and candidates with triage experience are highly desirable. Responsibilities: Provide Skilled Nursing Care in a home or facility setting Ensure the appropriate use of meds, DME, and supplies Educate and support patients, families, and caregivers Participate in IDT updates and updating the Plan of Care Provide our patients and families the best care possible Qualifications: Valid LVN or RN License in California One year or more of experience in either Home Health or Hospice is required Previous experience in palliative care and intake is a plus Ability to build rapport with patients Compassionate and caring demeanor Excellent written and verbal communication skills
