*Overview* Join our dynamic healthcare team as a Hospice Registered Nurse Case Manager, to support our patients around *South Orange County (Irvine, Laguna Hills, Laguna Woods, San Clemente and surrounding cities*_*)*_. The Hospice RN Case Manager is responsible for coordinating and managing the overall care of hospice patients and their families. This role ensures that patients receive high-quality, compassionate, and individualized end-of-life care in accordance with hospice philosophy, regulatory requirements, and physician orders. *Duties* * Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to determine care needs. * Develop individualized care plans in collaboration with multidisciplinary teams, focusing on care coordination. * Monitor patient progress through ongoing assessments, medical records review, and documentation of clinical findings in compliance with HIPAA regulations. * Collaborate with physicians, social workers, and other healthcare professionals to optimize patient care pathways. * Provide education on medical terminology, infection control practices, and physiology to patients and their families to promote understanding and engagement. * Provide field supervision to LVNs and Home Health Aides assigned to your patients * Perform field case management and other duties assigned *Requirements* * Registered Nurse with a valid state license * Minimum of 1 year experience as an RN required; hospice experience preferred * Ability to lift up to 50 pounds * Must clear criminal background check, physical, and drug screening * Valid driver's license, good driving record, and reliable transportation required * Willingness to drive to patient locations and work outside regular hours as needed * Experience working with diverse patient populations such as geriatrics, or patients requiring infectious disease care.
