*Overview* MGS Home Health is seeking a per diem Field Licensed Vocational Nurse (LVN) in the Santa Clarita, CA area. As a Field LVN, you will play an essential role in providing high-quality healthcare services to patients in the comfort of their homes. This position involves conducting patient assessments, delivering skilled nursing care, and assisting with case management, all while ensuring compliance with health regulations and industry standards. The ideal candidate will have at least 1 year of experience in Home Health and be skilled in providing compassionate, professional care in the home setting. *Responsibilities* * Conduct patient assessments to evaluate health status, needs, and care requirements. * Collaborate with Registered Nurses (RNs) and physicians to develop and implement individualized care plans. * Provide skilled nursing care such as wound care, medication management, and patient education. * Educate patients and families on self-care techniques, medication management, and lifestyle modifications. * Assist in coordinating care and maintaining communication with patients, family members, and healthcare providers to ensure continuity of care. * Monitor patient progress and report any changes to the RN or physician as needed. * Administer medications and treatments as prescribed, including injections and basic IV therapy, under the supervision of an RN or physician. * Document all patient interactions, assessments, and treatments accurately, in compliance with home health regulations and organizational standards. * Ensure infection control protocols are followed and maintain a safe and sterile environment for both patients and staff. * Lift and assist patients as necessary, ensuring their safety and comfort. *Skills* * At least 1 year of experience in Home Health nursing is required. * Proficient in performing nursing assessments and assisting in the development of home health care plans. * Skilled in wound care, medication management, and other home health nursing procedures. * Strong communication and teamwork skills, with the ability to collaborate effectively with RNs and other healthcare professionals. * Familiarity with home health documentation systems and legal requirements. * Ability to educate patients and families on managing health conditions and following care plans. * A valid California LVN license and any necessary certifications are required. *Please see rates below as follows:* _*LVN Follow-up visits = $40-$50 (depending on experience)*_ This is a per diem position for a compassionate and dedicated healthcare professional who is passionate about delivering high-quality, personalized care to patients in their homes.
