Responsibilities: Provide direct patient care in accordance with established care plans and protocols. Administer medications and treatments as prescribed by physicians. Monitor and document patients' vital signs and health status. Assist with activities of daily living (ADLs) to promote patient comfort. Collaborate with interdisciplinary teams to enhance patient care. Educate patients and families on care plans and health management. Participate in on-call duties as needed for patient support. Maintain accurate and timely documentation of patient care activities. Requirements: Current California LVN license with a strong understanding of palliative care principles. Minimum of 1 year of nursing experience, preferably in hospice or home health care. Strong communication skills and the ability to empathize with patients and families. Ability to work independently and manage time effectively. Basic Life Support (BLS) certification required. Valid driver's license and reliable transportation for patient visits. Proficient in electronic medical records (EMR) systems. Compassionate demeanor with a commitment to quality patient care.
