If you share the same passion for compassionate care, we invite you to apply for our *SHIFT NURSE Licensed Vocational Nurses (LVN) *position. *Why Join Us?* * *1:1 nurse-to-patient* ratios where you impact outcomes * *Flexible work schedules:,* We have opportunities for part-time, full-time, days, nights, or weekends. * *PTO,Sick Day, Holiday Pay, Medical/Dental/Vision Benefits* * *Weekly pay *with direct deposit * Continuing Education Classes * Referral program * *Joining Bonus* *Requirements:* * Current CA *state license* to practice as a *Licensed Vocational Nurse* in California. * A minimum of one (1) year home health experience is strongly preferred. * Current *BLS/CPR* Certification * Experience in caring for patients with *G-Tubes*, *Trachs,* and *ventilators.* * Works in *patients home* in various conditions; ability to work a flexible schedule. * Able to travel to prospective patient’s place of residence. * Able to stoop and bend; must be able to lift and transfer patients. *As a Home Health Licensed Vocational Nurse(LVN), what you’ll do:* *Responsibilities:* * Provide *home-based care* to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an *RN(Registered Nurse)* * *Licensed Vocational Nurse* ( LVNs)will be given a detailed plan of care instructed and administered by RN Case Manager * Care consists of *monitoring patient status*, maintaining activities of daily living and administering *medications.* * Take patient vitals, do assessments, document progress and report changes to RN supervisor. * Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques * Collaborates with physicians, other agency staff and contract services to coordinate and implement the *Plan of Care.* * Completes all required documentation in accordance with the standards of the agency, and regulatory requirements. *About Us* We are a licensed home healthcare provider in southern California, and we are contracted by the Regional Center and certified by Medicare.
