As a leading provider of hospice services, we are committed to supporting patients and families through a holistic, interdisciplinary approach. We are seeking a dedicated ( LVN )to join our care team. In this role, you will coordinate and provide high-quality, individualized care to terminally ill patients in their homes or residential care facilities. You will serve as the primary point of contact for patients and families, working collaboratively with physicians, social workers, chaplains, and other team members to support physical, emotional, and spiritual needs. EMR HOSPICEMD * Manage a caseload of hospice patients, providing skilled nursing care, routine assessments, and symptom management under RN supervision. * Contribute to the development and implementation of individualized plans of care in collaboration with the interdisciplinary team. * Provide direct patient care and perform regular visits based on patient needs and acuity. * Educate patients and families on disease process, medication administration, and comfort-focused care. * Coordinate care with the RN, hospice team members, and community providers to ensure continuity of care. * Maintain accurate, timely documentation in compliance with state and federal hospice regulations. * Report changes in patient condition promptly to the supervising RN. * Participate in on-call rotation as required. *Required:* * Current LVN license (in good standing) * Hospice or palliative care * Home health * Critical care * Valid driver’s license and reliable transportation * Current CPR certification * Experience with tracheostomy (trach) care Pay: From $90.00 per hour Work Location: Hybrid remote in Los Angeles, CA 90025
