*Location:* Los Angeles, DTLA *Employment Type:* Per Diem *Salary:* Start at $128.00/ Visit ( per-hour) *Experience Required:* Home Health Nursing Experience *Required* *Position Summary:* Aloha Healthcare is currently seeking a compassionate and skilled *Registered Nurse (RN)* with *previous home health experience* to join our team. In this role, you will provide direct nursing care to patients in their homes, ensuring quality and continuity of care as outlined in the individualized plan of care. The ideal candidate is self-motivated, organized, and able to work independently. *Responsibilities:* * Perform in-home skilled nursing visits in accordance with physician orders and agency protocols * Conduct comprehensive patient assessments, including physical, emotional, and environmental needs * Develop and update individualized care plans in collaboration with physicians and the interdisciplinary team * Administer medications and treatments as prescribed * Educate patients and caregivers on disease processes, medications, and self-care techniques * Accurately document all care provided in a timely and compliant manner * Monitor and report changes in patient condition to the physician and care team * Ensure compliance with all applicable federal, state, and local regulations * Maintain professional and ethical standards of nursing practice *Qualifications:* * Active and unrestricted *Registered Nurse (RN) license* in the state of California * *Minimum of 1 year of recent Home Health experience is required* * Current CPR certification * Reliable transportation and valid driver’s license * Strong clinical and critical thinking skills * Excellent communication, time management, and organizational skills * Familiarity with EMR systems (e.g., Kinnser, Homecare Homebase, or similar) is a plus *Preferred:* * Experience with OASIS documentation * Bilingual (optional, depending on service area) *Why Join Aloha Healthcare?* * Supportive and collaborative team environment * Flexible scheduling options * Competitive compensation * Opportunity to make a real difference in patients’ lives Pay: From $128.00 per hour Work Location: On the road
