Be part of an extraordinary life. *Essential Duties and Responsibilities* * Ability to follow verbal and written instructions. * Ability to be neat, clean and appropriately dressed to present an appearance acceptable to * residents and the public; in accordance with the employee handbook. * Ability to handle emergency situations in a calm, effective manner. * Ability to follow fire and disaster procedures during a drill and actual occurrence. * Ability to work cooperatively with all facility department supervisors and administration. * Ability to handle all correspondence assigned. * Ability to communicate and work cooperatively with facility staff. * Ability to accept assigned duties in a cooperative manner from your supervisor. * Ability to perform other related duties as assigned from your supervisor. * Ability to come to work as scheduled and consistently demonstrate dependability and punctuality. * Ability to keep charts up-to-date. * Ability to educate residents regarding health care issues, i.e. new medications. * Ability to administer flu shots annually. * Ability to supervise the performance of caregivers and communicate with the Coordinators and Director of all issues and potential disciplinary action that requires attention. * Ability to supervise caregivers and educate as needed of ADL and leisure activities as able to promote the resident’s highest ability to function and quality of life. * Ability to process recaps, reviewing and transferring all current orders to new month binders. * Ability to assess resident’s need for transfer to another level of care, addressing the health needs of the resident and in the development of the resident care plan with the intent to optimize the function, safety, health and emotional wellbeing of each resident. * Ability to assess and monitor the health needs of all residents to support the resident’s overall quality of life, safety and function. * Ability to communicate with Director of Health & Wellness on operations, and any issues involving residents, medication, and caregivers. * Ability to assist the Director of Health & Wellness in monitoring the wellbeing of residents. This includes noting changes in resident conditions, monitoring resident specialists and/or activity specialists care and notes, and following up on health-related issues noted in records. * Ability to recognize and interpret change in resident condition, notifies Coordinators, Director, and physician of condition change and takes appropriate actions as directed by physician, and families/guardian. * Ability to provide technical support to enable the dementia care team to execute the resident care plans. * Ability to do pharmacy audits and follow up on recommendation from pharmacist. * Ability to prepare all reports required by the Director and Coordinators * Ability to comply with the operation of Assisted Living and Memory Care within the guidelines of established facility policies and in compliance with federal, state and local regulations. * Ability to serve as an active member of all committees, where applicable. * Ability to facilitate and assist with physician visits. * Ability to communicate with physicians any change of condition, new orders or incidents. * Ability to process assessments, physician’s reports and care plans for each resident. * Ability to process medication management as ordered. * Ability to assess for pain, depression, anxiety, Chaplaincy need, etc. * Ability to act as a catalyst in developing friendships between residents and in promoting the social model. * Ability to be responsible for the household medication program and monitoring the appropriate usage of psychotropic medications and implementing the DRNO behavior modification protocol. * Ability to participate in QA program and to ensure optimum level of function, safety, health and wellbeing of all residents. * Ability to file incident reports in compliance with facility policy and notify the Director and Coordinators. * Ability to monitor resident nutritional intake and implement appropriate procedures. * Ability to monitor infection control procedures to ensure compliance to infection control policy. * Ability to monitor and assist, as needed, in administration and documentation of nursing observations and care provided, in nursing notes and specialists notes. * Ability to maintain on-going communication with Director of Health & Wellness. * Ability to monitor and order supplies as needed. * Ability to coordinate the resident’s internal and external healthcare services. * Ability to participate in staff education and in-service programs. * Responsible for maintaining resident dignity and respect. * All employees are responsible for accepting accountability for all data contained in the employee handbook. This includes the safety, infection control and resident’s rights policies and procedures. *Qualifications/Education/Experience:* * Relevant experience required * LVN License and CPR/BLS required * Must have the desire and ability to work with seniors with memory impairment in a social model of care. * Must be ambulatory and in sound physical condition, be able to lift up to 74 lbs., bend to the floor, reach above head level, sit, and perform repetitive movements with arms and legs. \*Professional references may be contacted prior to interview.
