Hello HOME HEALTH NURSES! LVN's needed ASAP! We have URGENT NEEDS for per diem/ per visit LVN's for Home Health to visit patients in Glendora, West Covina and surrounding areas. This is for IMMEDIATE HIRING- please apply : 1) if you can visit patients WEEKDAYS (not only weekends) 2) You can visit patients ASAP You can be hired this week and start visiting patients next week! We are Care Circle Home Health, a Joint Commission-accredited home health agency servicing various counties in SoCal. We offer flexible schedules, training, complete PPE’s and competitive pay. We also use user-friendly EMR/web-based documentation and we concentrate patient assignments close to our nurses’ homes to minimize time in traffic. You will be working with our highly qualified Clinical Management Team (RN Case Managers, Coordinators, dedicated QA Team) to provide quality direct patient care, such as: * Evaluating the patient, providing treatments and implementing the plan of care * providing accurate and timely documentation consistent with the plan of care * providing patient and family/caregiver education and important information relating to diagnosis and plan of care * actively participating in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietitian) to the clinician and/or clinical manager. *RN's * : at least 1 year LICENSED as an LVN, and with 1 year nursing experience (Required). Home Health: 1 year (Preferred) Bilingual English and Spanish a plus You will need to submit: * Current California-issued Professional License * Certificate for Basic Life Support or Active CPR Card * Driver's License and continue * Auto Insurance (with the applicant's name on the policy) * Physical examination within 6 months or "Fit to Work" doctor's note * PPD or TB/Chest X-Ray test result * Flu Shot record * Proof of Covid Vaccine Job Types: Part-time, Per diem Pay: $40.00 - $45.00 per hour Benefits: * Flexible schedule * Health insurance * Paid time off Medical Specialty: * Home Health Experience: * Nursing: 1 year (Required) License/Certification: * Professional License (Required) * State Nurse License (Required) Work Location: On the road
