*Job Overview* We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our home health care team. This is a per diem job covering the Kern County area and Ojai in Ventura County. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires strong clinical skills, excellent communication abilities, and a commitment to patient-centered care. *Responsibilities* * Deliver personalized nursing care to patients in their homes, including medication administration and monitoring vital signs. * Conduct thorough patient assessments and develop individualized care plans in collaboration with healthcare teams. * Provide support for activities of daily living (ADLs) and assist with mobility and personal hygiene. * Administer IV infusions, injections, and other medical treatments as prescribed. * Maintain accurate medical documentation and ensure compliance with HIPAA regulations. * Educate patients and families about health management, medication adherence, and disease prevention. * Collaborate with interdisciplinary teams to coordinate patient care and discharge planning. * Monitor patients for changes in condition and report significant findings to supervising nurses or physicians. *Skills* * Proficient in using EMR systems such as Kinnsear for medical documentation. * Strong knowledge of medical terminology, anatomy, physiology, and nursing practices. * Experience in various clinical settings, including acute care, long-term care, home health, hospice care, geriatrics, pediatrics, and emergency medicine. * Familiarity with critical care procedures such as airway management, catheterization, phlebotomy, IV insertion, and specimen collection/processing. * Ability to provide specialized care for patients with developmental disabilities, Alzheimer’s disease, dementia, or those requiring memory care. * Competence in pain management techniques, including acute pain management and medication administration for chronic conditions. * Excellent communication skills for effective patient education and family support. * Strong organizational skills with attention to detail for accurate patient assessments and documentation.
