You will be responsible for delivering personalized care plans, assessing patient needs, and collaborating with families and healthcare teams to ensure optimal outcomes. *Responsibilities* * Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations to inform care plans. * Administer medications via injections, IV infusions, and other routes while adhering strictly to HIPAA regulations and safety protocols. * Manage wound care procedures such as suturing, dressing changes, and sterile technique to promote healing. * Provide specialized care for patients with Alzheimer's disease, dementia, or memory impairments, ensuring safety and comfort. * Support patients with complex needs such as ventilator management, catheterization, tube feeding, and pain management strategies. * Document all patient interactions accurately using EMR (Electronic Medical Records) systems like Epic or Cerner; ensure compliance with HIPAA standards. * Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of post-acute or hospice care services. *Skills* * Proven experience with Epic EMR systems or similar EHR platforms such as Athenahealth or eClinicalWorks. * Critical care experience including ICU or Level I Trauma Center settings; hospital medicine or emergency room background preferred. * Strong knowledge of ICD-9/10 coding for accurate documentation and billing processes. * Expertise in working with individuals with developmental disabilities or those requiring specialized dementia or memory care. * Skilled in IV insertion, phlebotomy, medication administration, and sterile processing techniques. * Familiarity with infection control protocols and aseptic techniques essential for safe patient care. * Excellent patient assessment skills combined with physiology knowledge to evaluate acute pain management needs or infectious disease cases.
