*Job Summary* LVN FOR HOME HEALTH VISITS. Join our dedicated healthcare team as a Licensed Vocational Nurse (LVN) specializing in Home Health services! You will be responsible for delivering personalized care plans, assessing patient needs, and collaborating with families and healthcare teams to ensure optimal outcomes. *Responsibilities* * Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations to inform care plans. * Administer medications via injections, IV infusions, and other routes while adhering strictly to HIPAA regulations and safety protocols. * Manage wound care procedures such as suturing, dressing changes, and sterile technique to promote healing. * Provide specialized care for patients with Alzheimer's disease, dementia, or memory impairments, ensuring safety and comfort. * Support patients with complex needs such as ventilator management, catheterization, tube feeding, and pain management strategies. * Document all patient interactions accurately using EMR (Electronic Medical Records) systems like Epic or Cerner; ensure compliance with HIPAA standards. * Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of post-acute or hospice care services. *Skills* * Proven experience with Epic EMR systems or similar EHR platforms such as Athenahealth or eClinicalWorks. * Critical care experience including ICU or Level I Trauma Center settings; hospital medicine or emergency room background preferred. * Strong knowledge of ICD-9/10 coding for accurate documentation and billing processes. * Expertise in working with individuals with developmental disabilities or those requiring specialized dementia or memory care. * Skilled in IV insertion, phlebotomy, medication administration, and sterile processing techniques. * Familiarity with infection control protocols and aseptic techniques essential for safe patient care. * Excellent patient assessment skills combined with physiology knowledge to evaluate acute pain management needs or infectious disease cases.
