Our team values compassion, clinical excellence, and teamwork to deliver outstanding patient outcomes. *Position Summary* We are currently seeking compassionate and dependable *Licensed Vocational Nurses (LVNs) and Registered Nurses (RNs)* to join our home health team. Nurses will provide skilled care to patients in their homes, coordinate with the interdisciplinary team, and ensure high standards of clinical practice. *Responsibilities* For LVN: * Provide skilled nursing care under the supervision of an RN * Administer medications and treatments as ordered * Monitor patient condition and report changes * Perform wound care and other nursing procedures * Educate patients and caregivers * Complete accurate and timely documentation *RN) Responsibilities:* * Perform comprehensive initial and ongoing patient assessments to determine care needs * Develop, implement, and update individualized plans of care tailored to each patient * Provide skilled nursing services, including *IV therapy administration and management*, and supervise LVNs * Coordinate patient care with physicians and the interdisciplinary healthcare team * Conduct OASIS assessments and other required documentation (if applicable) * Ensure adherence to all state and federal regulations, standards of care, and organizational policies *Qualifications* * Current LVN or RN license * Home health experience preferred (not required) * Current CPR certification * Strong clinical assessment and communication skills * Reliable transportation and valid driver’s license * Ability to work independently and manage time effectively *Why Join Us?* * Competitive compensation * Flexible scheduling * Supportive leadership and team environment * Opportunity for professional growth * Make a meaningful impact in patients’ lives every day Job Types: Part-time, Travel nursing, PRN, Per diem Pay: $93,364.93 - $112,439.48 per year Benefits: * Flexible schedule Work Location: On the road
