A facility that provides long-term care services including sub-acute nursing care to residents, with licensed vocational nurses administering nursing care under registered nurse supervision, performing resident assessments, medication management, IV therapy, and participating in interdisciplinary team conferences to maintain compliance with regulatory agencies. The Licensed Vocational Nurse provides nursing care to designated residents under the supervision of a Registered Nurse. In addition, the LVN performs a variety of direct resident care as assigned by the Registered Nurse and/or Sub-Acute Program Manager. Essential responsibilities include administering nursing care as indicated by the nursing care plan, reviewing resident histories, assessing and planning nursing interventions, educating residents and families, monitoring resident conditions, initiating emergency measures including CPR, documenting resident care, managing medications and IV therapy, participating in team conferences, reporting abnormal test results, practicing infection control techniques, and serving as a resident advocate while maintaining a positive attitude and functioning as a role model for the care team.
