Registered Nurse (RN) – ECM Clinical Review & Assessment Mysti’s Adult & Family Services is seeking a Registered Nurse (RN) to provide clinical oversight for Enhanced Care Management (ECM) cases. This role focuses on reviewing and signing care plans, completing client assessments, and conducting periodic clinical check-ins to ensure care plans remain appropriate, compliant, and client-centered. Responsibilities include reviewing and approving ECM care plans, conducting nursing assessments (phone, virtual, or in-person as needed), checking in with clients regarding health status and changes, identifying clinical risks, and providing clinical guidance to care teams. The RN will document assessments and recommendations in accordance with Medi-Cal and ECM requirements. This is not a bedside or hands-on nursing role. It does not include case management, administrative duties, or intake functions. The role is primarily remote or hybrid.
