The MDS Coordinator (LPN, RN) is responsible for overseeing the resident assessment and care planning process and ensuring compliance with federal and state regulations related to resident assessments, quality of care and Medicare/Medicaid reimbursement. Key responsibilities include conducting and completing the Minimum Data Set (MDS) assessment to evaluate residents' physical, psychological and functional status, evaluating each resident's condition and pertinent medical data to determine any need for special assessment activities, preparing and electronically transmitting timely reports to national Medicare and Medicaid databases, developing a written plan of care for each resident that identifies problems/needs and goals, providing information to residents/families on Medicare/Medicaid and other financial assistance programs, ensuring that MDS notes are informative and descriptive of services provided and residents' response to service, and assisting with completing the care plan portion of residents' discharge plan.
