Skilled Nursing Facility offering positions for Registered Nurses to deliver direct patient care including medications, treatments, vital signs monitoring, personalized care planning, coordination with healthcare teams, documentation, assistance with daily living activities, patient and family education, and emergency response. The facility emphasizes compassionate care following physician's orders and guidelines, with a focus on meeting residents' emotional, physical, and cognitive needs. As a Registered Nurse (RN) at a Skilled Nursing Facility, you will ensure residents receive care following best standards and physician's orders. You will deliver direct patient care including medications, treatments, and vital signs monitoring; create personalized care plans; coordinate with healthcare team members; maintain accurate documentation; assist with daily living activities; educate residents and families about healthcare; respond to emergencies with first aid; and adhere to facility guidelines. The role requires a valid California RN license, proficiency with medication administration, graduation from an accredited nursing program, at least 1 year of experience in skilled nursing facility or acute care settings, strong clinical and communication skills, and experience with clinical software systems like American Health Tech (AHT).
