Memory Care Support Partner responsible for understanding the importance of empathy and creating human connection with residents during their most vulnerable moments. The role involves discovering residents' interests and abilities, collaborating to create appropriate engagement plans, setting the stage for successful days through environmental preparation, partnering with residents through all facets of daily life, documenting resident care and changes in condition, and assisting with orientation of team members assigned to memory care neighborhoods.
