The Care Manager/""Designated Care Manager"" is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe and comfortable home like environment. Essential duties include reviewing daily logs, participating in ISP development, monitoring resident conditions, responding to call bells, assisting with activities of daily living, maintaining hygiene standards, ensuring safety compliance, providing housekeeping and laundry services, supporting dining services, and engaging residents in life enrichment activities. The role requires a High School diploma/GED, CPR and First Aid certification, ability to work with seniors, and willingness to serve and care for seniors.
