The Care Manager/""Designated Care Manager"" is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe and comfortable home like environment. Essential duties include reviewing daily logs, participating in ISP development, monitoring residents' conditions, responding to call bells, assisting with activities of daily living, maintaining resident rooms and laundry, serving meals, and engaging residents in life enrichment activities. The role requires CPR and First Aid certification, high school diploma/GED, ability to work calmly under pressure, and previous experience with seniors preferred.
