The Care Manager/""Designated Care Manager"" is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in assisted living/long term care and reminiscence neighborhoods. The role includes providing hands-on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe and comfortable home-like environment. Essential duties encompass resident care including reviewing daily logs, participating in ISP development, monitoring resident conditions, responding to call bells, assisting with activities of daily living, maintaining housekeeping and laundry services, supporting dining services, and contributing to team success through training and engagement. The position requires a High School diploma/GED, CPR Certificate and First Aid certification, minimum age of 18, and a desire to serve seniors.
