Assist the men with scheduling and coordination of health care specialists; Accompany Jesuits to physician or specialist appointments as needed; Report changes in condition and relevant health updates to the nurse on duty, Care Coordinator Nurse, and Jesuit leadership; Manage admissions and readmission, including communication of condition, treatments, and medications; Ensure care plans are accurately and promptly communicated to the Care Coordinator Nurse and/or floor nurse on duty; Assume full responsibility for assigned cases; Complete documentation, charting, and planning in accordance with prescribed procedures
