The Care Manager/""Designated Care Manager"" is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe and comfortable home like environment. Essential duties include reviewing daily logs, participating in development of ISPs, monitoring resident conditions, responding to call bells promptly, assisting with activities of daily living, maintaining cleanliness and safety, serving meals, engaging residents in life enrichment activities, and participating in training and team meetings. The role requires a High School diploma/GED, CPR Certificate and First Aid certification, ability to remain calm in difficult situations, good communication skills, and a desire to serve seniors.
