Conducts comprehensive OASIS initial nursing assessments, including physical, emotional, and mental needs. Determines eligibility for services related to homebound status and need for skilled services. Develops an individualized plan of care in consultation with the patient's physician and other agency personnel. Develops and implements the care plan for the patient and family. Obtains specimens as ordered by physician and submits to laboratory. Participates in team meetings and patient care conferences. Provides direct nursing care procedures to patients and updates primary physician when necessary and at least every 60 days. Completes OASIS at all points of care as required. Responsible for thorough documentation to ensure continuity of care and maintenance of accurate legal documents. Teaches family members and significant others patient care skills. Participates in on-call duties. Ensures arrangements for equipment and necessary items are available. Instructs, supervises, and evaluates home health aide care provided at least every two weeks. Attends in-services and department meetings as required.
