EverTrue is a community that provides medication administration, supportive and functional care to residents with routine daily care in accordance with established community care procedures, operating within CMT scope of practice and state and federal regulations. Provides medication administration to residents under the direction of the nursing staff. Provides supportive and functional care to residents with routine daily care in accordance with established community care procedures as directed by nursing leadership. Works as a member of and collaboratively with the interdisciplinary team with a focus on maintaining the resident as the director of their care. Prepares and administers medication to residents under the direction of the nursing staff and within the scope of practice for a CMT; observes residents for side effects, interactions and implications of common medicines. Provides medical and physical care to residents to meet their daily needs. Handles residents' personal care needs and support in a timely manner. Observes residents closely and identifies changing needs and conditions. Actively participates in the resident's care/service plan. Assists residents during mealtimes. Creates and maintains meaningful relationships with residents and families. Supports resident involvement in decision making. Assists with therapies and treatments under licensed personnel direction. Respects privacy and HIPAA requirements. Follows all safety procedures and protocols. Documents vital signs and pertinent information in electronic health records. May serve as administrator designee if 21 years or older.
