LPN/RN MDS Coordinator role at Beauvais Rehab and Healthcare Center responsible for conducting and coordinating Resident Assessment Instruments (RAIs) per federal, state, and local regulations, collaborating with the Interdisciplinary Care Plan Team to develop resident-specific care plans, evaluating resident conditions and ensuring all special assessments are completed as required, and providing data for the Facility's Quality Assurance Program.
