This is a Full-time position during the evening shift from Noon -8:30 pm. Duties include evaluating potential Hospice patients, maintaining communication with physicians and facilities, coordinating hospice services, completing admission processes, and potentially providing nursing care to Hospice patients. Requirements include a current RN license in CA, minimum one year acute-care experience, with hospice and EMR experience preferred.
