Job Description
1. Assists residents to mealtime, activities, and outings as necessary. Assists residents with Activities of Daily Living (ADLs) as indicated on the Individualized Service Plans (ISP), reviewing this plan daily. 2. Completes and signs off on Daily Assignments to include dressing, bathing, grooming, hygiene, escorts, status checks, meal attendance, incontinence assistance/toileting, and other daily assigned tasks. 3. Communicates clearly with residents, and notifies Med Tech or Wellness Coordinator of any changes in condition or concerns and records those changes of condition or concerns in communication logs as appropriate. 4. Monitor residents and provide regular room and facility grounds checks to ensure resident health and safety. Report any housekeeping or maintenance requirements to Med Tech or Wellness Coordinator. 5. Assist or lead activities as assigned by Activity Coordinator, enabling and supporting residents’ independence. 6. Other duties as may be assigned.
