Job Description
"Green Hill Rehab and Nursing Care Center is a skilled nursing facility seeking Licensed Practical Nurses, providing coordinated care to residents under the leadership of a Registered Nurse. Under the leadership of the Registered Nurse, the LPN will be providing coordinated care to residents in a skilled nursing facility. Care Plans: Assists the attending physician, Transcribes, documents and implements physician's orders, Contacts the resident's physician and family, Medication Pass, Treatments, and other medical necessities, Responsible for completing required routine documentation."
